Tempe St. Luke’s Hospital maintains medical records for each patient treated at the hospital. To request a copy of your medical records, please contact our Health Information Management office between the hours of 8:00 a.m. - 5:00 p.m., Monday through Friday at (480) 784-5567. 

Prior to receiving your records, you will be asked to complete a medical records release authorization form and to provide proof of your identification with a valid government issued photo ID. This form along with a copy of your proof of identification can be faxed to (480) 784-5630.

Records needed for continuing care purposes will be sent directly to your provider.

Requests for medical records generally take up to 72 hours to process.
